Add Health & Safety Screening Questions to Phone-In or SMS Clock-Ins in VeriClock

This guide explains how to require structured health, safety, or compliance confirmations when employees clock in through phone (IVR) or SMS. By using custom list-type fields, administrators can enforce site-specific screening questions—helping organizations document policy acknowledgment, safety checks, or regulatory compliance at the time of entry.

In your account go to your tabs Settings-->Custom Fields

• Step 1) To set up a custom list field for call in from a phone, click--> Add List Field  
• Step 2) Click on the newly created Custom Field to highlight.
• Step 3) Under Field Settings click the box beside Name. Here you will enter the question and instructions for the caller. This is what the caller will hear.                                             
  • Note: this is a limited amount of characters. At the bottom of the page are some sample questions. Questions will need to include instructions on what to press next.
• Step 4) Click --> Edit Item List.  Add Codes for the phone: Code: 1, Name: No, Click --> Add Item. Next Line enter Code: 2 Name: Yes, Click--> Add Item, Click --> Close 

• Step 5) Choose when and what clock in / clock out methods will be using these questions. Choose if you want the employees to hear on clock in and/or clock out. Choose phone for phone-in (IVR)
• Step 6) Choose Required or Optional
• Step 7) Check mark on Turn field on for everyone will apply the field to every employee, group, and job that uses the phone-in option. Uncheck the box to enable specific Groups, Jobs or Employees
• Click Save, to save any custom fields before navigating away from this page.

Sample COVID Health Screening Questions

Employees using voice call in to clock in are not able to hear their “list items” that they need to choose from unless they are included with the question ie. "press 1 for no, 2 for yes”. The name field has a limited character limit.These are some pre-created examples for steps 3 & 4

Do you or someone in your household have any Covid-19 symptoms? 1 for no, 2 for yes

Under the Edit List Item button, add these:

Code: 1   Name: No

Code: 2    Name: Yes

Have you traveled outside of the country within the last 14 days? Press 1 for no or 2 for yes

Under the Edit List Item button, add these:

Code: 1   Name: No

Code: 2    Name: Yes

Have you traveled outside of the state within the last 14 days? Press 1 for no or 2 for yes

Under the Edit List Item button, add these:

Have you been in contact with anyone who has tested positive for Covid-19? Press 1 for no, 2 for yes

Under the Edit List Item button, add these:
